Bank feed is active but not pulling in entries, and does not respond to refresh request.

Have you tried removing the feed and re adding it?, this usually resolves any issues with authorisations on the feed.
Once you have re set up the feed it should allow you to refresh it back 21 days.
For any transactions older than this you would have to [import a bank statement].(Importing your bank statement)
